RELATED PERSON APPLICATION FORM UNDER THE PERSONAL DATA PROTECTION LAW No. 6698
Personal data owners defined as “relevant persons” (referred to as “Applicants”) in the Personal Data Protection Law No. 6698 (“KVKK”) are granted the following rights within the scope of Article 11 of the KVKK:
• Learning whether your personal data is being processed or not,
• Requesting information if your personal data has been processed,
• Learning the purpose of processing your personal data and whether they are used for their intended purpose,
• Knowing the third parties to whom your personal data is transferred at home or abroad,
• Requesting correction of your personal data if it has been processed incompletely or incorrectly, and requesting that the action taken in this context be notified to third parties to whom your personal data has been transferred,
• Requesting the deletion or destruction of your personal data in case the reasons requiring processing are eliminated, even though it has been processed in accordance with KVKK and other laws and other legislation provisions, and requesting that the action taken in this context be notified to third parties to whom your personal data has been transferred,
• Object to the emergence of a result against you by analyzing the processed data exclusively through automatic systems,
• Request compensation for the damage if you suffer damage due to unlawful processing of your personal data.
In accordance with the 1st paragraph of Article 13 of the KVKK and the Communiqué on the Procedures and Principles of Application to the Data Controller, applications to be made to our Company regarding these rights can be made in writing or by using the e-mail address previously notified to our Company by you and registered in our Company’s system at info@ It must be transmitted by sending an e-mail to bilgilersogutma.com.tr or by other methods to be determined in the future by the Personal Data Protection Board (referred to as the “Board”).
In this context, applications to be made to our Company in “written” form, by printing this form;
• By applying to our Company personally by the Applicant,
• Through a notary public or by registered letter,
• If the Applicant has an e-mail address registered in our Company’s systems, by sending an e-mail from this address to our Company’s address info@erdemlersogutma.com.tr,
• It can be transmitted by signing by the Applicant with the “secure electronic signature” defined in the Electronic Signature Law No. 5070 and sending it to the Company’s registered e-mail address.
Below, information is given specific to written application channels on how written applications can be submitted to us:

In addition, after the other methods determined by the Board are announced, our Company will announce how applications will be received through these methods.
Your applications submitted to us will be answered within thirty days from the date your request reaches us, depending on the nature of the request, in accordance with the 2nd paragraph of Article 13 of the KVKK. Our responses will be delivered to you in writing or electronically in accordance with the provision of Article 13 of the KVKK.
